Key Points Question Can technology-augmented behavioral counseling reduce risky firearm behaviors (RFBs) and associated outcomes in adolescents and emerging adults carrying firearms? Findings In this randomized clinical trial conducted among 373 patients presenting to the emergency department reporting firearm carriage, behavioral counseling combining motivational interviewing and care management supplemented by tailored digital application support provided over 30 days reduced RFBs, as well as other violence (experiencing violence, violent injury, consequences), criminal justice, substance use, and mental health (anxiety, depression) outcomes during the 6-month follow-up period. Meaning Technology-augmented behavioral counseling is a promising clinical strategy for reducing RFBs and associated clinical and behavioral outcomes among at-risk adolescent and emerging adult populations.
